Hasselmo hall second floor water event 9/25/2013


At 7:05 am the call center called Karen Collins with a report of leak in 1-202.

 Staff was called immediately and the water was shut off and cleanup ensued.   A water hose connection for a dishwasher in that room 2-206 had ruptured.   Water ran out into the hallway and into other rooms and also into rooms below. .

Rooms affected were2-202,  2-206, 2-210, 2-230, and 1-202, 1-205, 1-209, 1-208, 1-208B, 1-209,
1-212.

Departmental equipment was covered, although some had already gotten wet.  Departments will check equipment.  Some sheetrock and ceiling tile replacement may be required.

Chain of escalation:

Customer called call center, they called Team manager and staff responded and shut water off and
cleaned up area.

Tim Lahti contacted Neil Carlson, DEHS at approx. 7:15 am by phone.  N. Carlson was off site and contacted Mike Buck at DEHS Boynton.

Approx. 7:30am:  EHS  Mike Buck responded along with Greg Casura and David Paulu.  Neil Carlson at approx. 8:15 am.  AHC Cheryl Miller came in.  Customers in area were notified.  Water was checked and cleared for chemical and radiation hazards.

Greg Casura personally notified Nick Greco DEM at 8:50am prior to the Gopher Game day meeting at TCF bank stadium.

Mavo to provide fans and dehumidifiers.

Causal Factors:

A di water hose for a dishwasher in room 2-206 ruptured.  This gravity feed dishwasher had recently been moved from another building and was installed in this room.

Action taken to remediate:

Water cleaned up.  Departments notified and they are accessing their equipment.

What could have been done to avoid the situation:

 The dishwasher needs to be checked to be sure that it is appropriate for the pressurized dionized
system in Hasselmo.

Actions to be taken to avoid situation from occurring in the future:

Specs for equipment that is relocated should be taken into account.








Cheryl Miller, Karen Collins, Neil Carlson, and Michael Buck are currently meeting to check walls and cabinetry for moisture. Below is a list of all the rooms affected by this morning’s water incident, along with some notes on what has been – or may be – affected:

1st floor hallway
·         Walls lining hallway should be checked for moisture
·         2 ceiling tiles removed in hallway
1-202
·         Cabinetry along west wall of room hit with water along base
·         14 ceiling tiles have water damage
·         2 light fixtures have water collecting in them
·         Equipment likely okay but should check back with lab later
·         Fume hood likely damaged; wall behind hood to be checked for moisture
1-202A
·         Water traveled in right near door; check walls for moisture
1-203
·         No water but saw walls were marked for removal, perhaps leftover from last water event?
1-205 Main Space
·         Carpet near middle of north wall and near door to conference room hit with dripping water; wet spot approx. 4-5 ft in diameter
·         Water leaked onto desk in main space near SW side of room; water wiped up off desk but should be checked for any warping
·         10 ceiling tiles affected by dripping water; line of them along North wall and 1 tile above desk
·         1 office chair soaked with water; other chairs stayed dry
·         North wall near door to conference room to be checked for moisture as well as wall near door to 1-209
1-205 Conference Room (no room official room number)
·         3 ceiling tiles wet
·         Small spot of carpet got wet directly under ceiling tiles
·         Water in nearby light fixture
1-208B
·         Equipment hit with water; damage TBD by lab once equipment dries out
1-209
·         5 ceiling tiles wet
·         Water dripped on desk in this room; check for warping later
·         Wall-mounted panel above desk got a little wet
·         Carpet got wet – small portion, NE corner of room and around door
1-212
·         Some ceiling tiles need to be replaced
1-214
·         Instrumentation on desk near 1-208B was hit with water, damage TBD by lab
·         Other equipment appears to be okay but lab will confirm

2nd Floor Hallway
·         Walls lining hallway to be checked for moisture
·         2 ceiling tiles removed in hallway
2-202A
·         Floor near walls adjacent to entrance to room got wet; walls to be checked for moisture
·         Water seeped under incubator; lab should check to ensure it still works properly
2-202 Biohazard Room
·         Water entered space; surrounding walls to be checked for moisture
2-202B
·         2 cabinets under lab bench soaked with water at base
·         Surrounding walls to be checked for moisture
2-205
·         Water along North & South walls; walls to be checked for moisture
·         Sensitive lab research materials in this room; no chemicals or disturbances should happen in this room without informing lab first
·         Box of media dishes under lab benches soaked but wrapped in plastic
2-206
·         Location of dishwasher that caused leak
·         2 boxes of plastic lab supplies got wet
·         Bottoms of 2 broken glass containers got wet: 1 of these containers has glass in it
2-209
·         Water on floor in this room; walls to be checked for moisture
2-209A
·         Water on floor in this room; walls to be checked for moisture
2-210
·         Base of 2 bookshelves outside 2-210A got wet
·         Check surrounding walls for moisture
·         Bases of cabinetry got wet
·         Water around base of Fumehood 011
·         Bottom of unopened package under computer soaked
·         Box of towels/bibs by bench 6 soaked and will need to be replaced

·         Water lining both walls to green double doors (east side); walls to be checked for moisture
·         Water under equipment but unlikely to be an issue
2-211A
·         Water lining both North & South walls; walls to be checked for moisture
2-220A
·         Check walls adjacent to door for moisture
2-220B
·         Check surrounding walls for moisture
·         Bases of 4 cabinets under lab benches soaked
2-220 Coat Closet
·         Floors hit with water; check surrounding walls for moisture
2-230
·         Cabinet for sink and adjacent wall next to door leading to 2-236 to be checked for moisture
·         Water along base of cabinetry in alcove near biosafety cabinet
·         Water along middle cabinetry in room
2-221
·         Water lining walls in room; walls to be checked for moisture

·         Box full of boxes of Accuwipes soaked and needing to be replaced
