Chronology:
6:15 am: Simplex called with a fire trouble. The fire department was called and cleared the alarm but recommended an electrician come in. Les found a head that was wet in room 1-212 and investigated and found water coming out from under the door in 2-219. A water hose connection in that room had come loose and water was spraying over the counter and ran out into the hallway and into other rooms. Rooms affected were 2-206, 2-210, 2-220a, 2-230 and 2-219. The damage was incidental, boxes of supplies, counter base and other things left on the floor may have gotten wet in these rooms. Wall damage is probably minimal due to the coved flooring in these labs. Departmental equipment did not appear to be wet. On the first floor, some water leaked through the ceiling in areas, rooms 1-202 (departmental equipment and chairs wet), 1-209A (wall damage) and 1-212 (departmental equipment).

Cleanup was mostly completed by 10:00 am, but mechanics and custodial staff will revisit area later today as water is still dripping in some places.

EHS to assess water damage on 7/8/2013.
After Les reported the water, mechanics Dale Runge, Bill Allen and custodians Bill Dow and Dillon
Murray brought wet vacs. Building manager Cheryl Miller was called and she came in and is contacting lab personnel. Called BSAC, police and EHS to report water incident. Talked to Nick Greco with DEM, Kathy Berlin AHC office of emergency response, Neil Carlson and Mike Austin with EHS, Tucker LeBien
7/8/2012:
0915 EHS Neil Carlson and Kelly Mullane assessed second floor and part of first. Water damaged walls were marked. Occupants were told to discard water damaged cardboard.
1049 Neil Carlson sends email via phone noting that walls are marked for water damage on the second floor and that additional shop vacs will be needed to remove water seeping out from under the cabinets.
15:02 Cheryl Miller requested additional assessment of first floor by DEHS:
We need to have 1st looked at as well. There is a wall on first that was wet in the BIPL area. There is a hood on first that has water in it, and tiles that will need to be replaced on 1st where water seems to have come through. I think we should also check the walls by the sequencer in Genomics and perhaps the chiller room in EM just to be sure.
DEHS will followup on 7/9/2013 am.
Cause:
A water hose connection in room 2-219 had come loose and water was spraying over the counter and ran out into the hallway and into other rooms.
Millipore filtration system - infrequently used in the past was connected to tap water.
The break was at the connection between the pressure gauge and the white tubing

Immediate action:
Water cleaned up. Departments notified and they are accessing their equipment.
What could have been done to avoid the situation:
Lab personnel need to be cautioned about leaving water on for the long weekends and connections need to be checked.
Actions to be taken to avoid situation from occurring in the future:
In the future, when the fire department clears an alarm, it may be prudent for the mechanics to double check the device. The electrician was on site within a half hour of the call, so this really wouldn’t have made much of a difference in this instance. Communication with lab personnel is important.
